How to register
To register for our online services you will need to complete a paper form which can be collected from our Reception. Please find the online access application forms below, you can print and complete them before coming into the surgery with photo ID.
We will need to verify your I.D in person, or alternatively you can verify your I.D through the NHS app if you have a smartphone, we will then issue you a username and password.
You may still be asked for proof of I.D and address if necessary.

Once you are registered you will be able to use the service to:
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- Change your contact details
- Review your medications and known allergies
